Message to Applicants

The Board has no way of knowing you except through this application. Therefore, we would like you to tell us your “story” and include a photo of yourself (or family) so that we might better understand your need for our assistance. Please attach a photo and a one page, typed or printed story to your application. Photographs will be returned. Once completed, submit

* your printed “story”
* a copy of your pathology report

The following patients have applied for financial assistance. Please take some time to read their stories. Brain cancer does not discriminate. Any of these stories could be about you or someone you love.
 
 
Kilee – OH
 
  Kilee was not immediately diagnosed with a brain tumor until she suddenly couldn’t walk.  A CT scan finally showed the large mass in the middle of her brain.  To everyone’s surprise, she came through surgery and just recently got enough strength back to move around.  This two year old has come a long way and her family continues to pray for miracles.  
 
 
Doryan – CA
 
 
Doryan was diagnosed with a stage 4 brain tumor at 14 months old.  Since then he has received chemo treatment and a stem cell transplant.  He has needed to be hospitalized many times and is a very brave little boy.  His mother and three siblings are staying positive and looking forward to his 3rd birthday which will take place this month (September 2010).  Doryan’s mother JoGina needs help with household expenses.
 
 
 
Samuel – DE
 
 
Samuel had always taken care of himself, eating healthy and staying active.  He was always a hard worker and never needed a doctor for anything.  But on April 6th, his life changed.  He had a seizure at work and was taken to the hospital.  Tests showed a large brain tumor.  Samuel can no longer live independently and his mother and stepfather provide 24 hour supervision.  He is hoping for the best and trying to keep a positive attitude despite life’s challenges.
 
 
 
Danielle – AZ
 
  Danielle (Dani) was a beautiful 14 year old, loving life and all that it offers when suddenly she was diagnosed with a brain tumor in November of 2008.  Now this beautiful 16 year old is home-bound and sad.  She lost contact with all of her friends.  She has been through chemo and will now start radiation. The trip to receive treatment is 60 miles.  Dani’s family hopes that this will be the answer.  
 
 
James – NJ
 
  Only 50 people in the whole country get this type of rare brain tumor each year.  James often says, “I went to sleep and woke up a month later in the hospital.”  Radiation, chemo and surgery have left him with cognitive impairment.  He now has little or no memory of his illness.  He can’t drive.  He doesn’t date.  He feels different but now doesn’t understand why.  He has emotional issues and lacks socialization skills.  James’s illness has put tremendous financial strain on his parents and the medical bills are draining all their resources.  
 
 
Ashley – ME
 
  Ashley (wearing the sweatshirt and glasses) is Mission4Maureen’s first applicant from Maine.  Also pictured here are her mother Erica, her father Craig, and siblings Olivia and Justin.  Ashley’s mother and father are unable to work due to their own medical issues.  Her brother and sister are in good health.  Ashley, age 16, was diagnosed with a brain tumor a week before Christmas in 2009.  The left side of her body has been effected.  Transportation to and from Ashley’s doctor appointments is this family’s most important need at the present time.  
 
 
Daniel – AL
 
 
Daniel was diagnosed with grade IV glioblastoma and several astrocytomas that were inoperable.  He is 21 years old and a first time father of a 1 year old boy.  He and his wife, Haley are trying to get by, day by day.  He receives SSDI but it barely covers rent and utilities.  They are relying on family but things are hard on everyone these days.  He prays each night for God to heal him and tries to live each day to its fullest enjoying the people he loves.  He has a younger brother Matthew who is 8 years old and they read a bible verse together… “But Jesus beheld them, and said unto them, with men this is impossible but with God, all things are possible.”  So each day is another day and this is the positive energy Daniel surrounds himself with.
 
 
 
Brianna - TX
 
  Brianna’s mom, Angel, is a single mother of five children.  In December of 2009, she was told that her middle child, Brianna (age 4), had brain tumor.  The tumor was inoperable so radiation was the recommended treatment.  She needed to stay in the hospital for a month.  During this time, Angel lost her job.  She has now been unemployed for almost 8 months.  Angel has turned to Mission4Maureen for help with her auto payments, so that she can at least get her children to school and get Brianna to her treatments.  
 
 
Carlos – MA
 
  Carlos is originally from El Salvador.  He came to the United States in 1993 with Temporary Protected Status because of the civil war in his country.  He and his wife have two daughters.  Carlos worked hard in construction and then on a recycling truck 6 days a week.  His wife worked in a local fast food restaurant.  Last September he started to get horrible headaches.  His doctor sent him to a hospital in Boston where his brain cancer was discovered.  Carlos is on disability and his wife had to quit her job.  There is not enough income coming in for all the bills that must go out.  He is always worried about paying for rent, utilities, food and all the things that his daughters need.  
 
 
Delilah – TX
 
  Delilah is 64 years old and has lived in East Texas for her whole lifetime.  She was diagnosed with glioblastoma multiforme on June 5, 2009.  She originally had trouble using her hand.  After having surgery for both carpel tunnel and for Dupuytren’s contracture (the drawing inward of her fingers), her hand was no better. Then she began having problems speaking.  This led to an MRI and the tumor was finally discovered.  So as it turned out, both surgeries on her hand were not necessary and since the surgeries, and she has never regained the use of her hand.  She and her husband have already paid out thousands of dollars out-of-pocket and have exceeded her lifetime maximum payout.  Her husband is retired and is unable to supplement their retirement income because of the need to care for Delilah full time.  
 
 
Usher – NY
 
  This family’s financial situation at this time is really tight.  When the head of a household is physically sick he/she is often unable to work.  Not only is there no income but expenses increase as well. The father of this family, Usher, was the one who drove.  Now there are transportation problems even getting him to his doctor appointments.  His brain cancer illness has been stressful and exhausting.  The expenses that have accumulated along with the necessary equipment needed in the home for a disabled person have stretched the family budget to its limit.  The family is trying to get by, day by day and is hoping that Mission4Maureen can do something – anything to help them.  
 
 
Kathy – GA
 
  Kathy is 54 years old and was diagnosed with glioblastoma multiforme in April of this year.  The doctors say that her prognosis is bleak but she is fighting back with surgery, radiation and chemotherapy.  She cannot drive and can no longer work due to occasional seizures and must rely on others for everything.  Unemployment benefits are her sole source of income.  She is hoping that Mission4Maureen can help her stay in her home by aiding her with payment of a few key household bills.  
 
 
Grace – MI
 
  Grace is a 64 year old woman who lives in Michigan. She was diagnosed with cancer in 2007. After surgery, chemotherapy and radiation, she was doing well and her check-ups with her Oncologist were good. Then in February of 2010 she began feeling under the weather and ended up in an emergency room where an MRI revealed six brain tumors. Grace was a home health aide for 8 years and loved her job. She has now had to stop working which has left her in extreme financial hardship.
