Brain cancer patients who wish to apply to Mission4Maureen for financial assistance should complete and electronically submit the application presented here.
Message to Applicants
In order to submit an application you must have the following available:
* an email address
* digital photo of yourself or your family (to be uploaded)
* a copy of your pathology/MRI report (to be uploaded)
The Board has no way of knowing you except through this application. Therefore, we request that you provide basic personal and financial information and that you tell us your “story” so that we might better understand your need for our assistance.
Mission4Maureen has redesigned the application procedure in order to reduce costs and to potentially speed up the application verification and review process.
Please note: Mission4Maureen provides assistance by paying bills on behalf of qualified patients and their families. |
